What is breast revision surgery?
Cosmetic breast revision surgery is a specialty that requires years of practice before a surgeon can produce consistent results. This specialty is becoming more common due to the increase in breast surgery patients over the last 40 years. Complications requiring revision occur less than two percent among patients. It takes a busy, successful surgeon with at least 20 years in practice to have seen and treated enough patients to gather the experience to be able to address most all of the problems one may see in a lifetime of surgical practice.

What are the reasons for breast revision surgery?
Among the most common reasons for cosmetic breast revision surgery are:
Implant Malposition: when a breast implant that is sitting on the patient’s chest wall in the wrong location. The implant can either be too high or too low, the implants can be too wide (or far apart), or too close together.
Capsular Contracture: Capsular contracture, or hardening of the breast due to the formation of contracted scar tissue around the implant, is less common today than in the first decades of breast implantation dating from the late 1960’s.
Saline Breast Implant Leaking: we see a much higher failure rate than we do with silicone breast implants. This is due to the lack of support strength provided by the shell or wall of the implant. The saline or salt water does not provide enough support, compared to silicone, to keep the implant from folding in on itself.
Silicone Breast Implant Rupture: A leaking silicone breast implant is much harder to detect than a leaking saline breast implant. Whereas loss of volume is easily noticed when a saline implant leaks, one does not really see any volume loss from a leaking silicone implant. Leaking silicone implants are usually noted during mammogram or MRI evaluation of the breasts. First and foremost, a leaking silicone breast implant will NOT affect your health, no matter what you may have seen or heard in the press or on the Internet. Liquid silicone does NOT drift to other areas of your body and cause medical problems. Once a silicone breast implant shell is ruptured, the silicone stays right where it was originally placed in the breast pocket. Very, very rarely liquid silicone gel can migrate into the armpit.
Breast Implant Rippling: All breast implants have some degree of rippling including saline or silicone, and round, teardrop, textured or smooth. Different implants have different amounts of rippling depending on the materials used. Here’s a list of implants in order of their risk for rippling (least to most):
- Smooth Silicone (least rippling)
- Textured Silicone
- Smooth Saline
- Textured Saline Teardrop
- Textured Saline Round (most rippling)
Other complications may include:
- Breast Implant Infection
- Implants that are either too small or too large
- Continued breast tissue sagging after a lift or reduction
- Nipples in the wrong place after breast lift or reduction
How can problems with cosmetic breast surgery be corrected?
Surgery is not an exact science and unforeseen complications may occur. Problems like deflation of saline implants or rupture of silicone implants or other medical complication are infrequent, but on occasion they happen. If any problem arises, it is important to have a surgeon who stands with the patient and is willing to do what is necessary with a remedy.
